№ 140321/2908/022 «Оказание услуг по налоговой сертификации на 2014 год».
РЕШИЛИ:
На основании п. 20.5.2 Единого отраслевого стандарта закупок (Положения о закупках) Госкорпорации «Росатом» согласно Извещению (регистрационный № 140321/2908/022 Госкорпорации «Росатом»), размещенному на сайте Госкорпорации «Росатом» http://zakupki.rosatom.ru от 21.03.2014 г. на оказание услуг по налоговой сертификации на 2014 год, было принято решение о внесении изменений в документацию по запросу цен (в перевод), а именно:Перевод формы 1 раздела 5 «ЗАЯВКА НА УЧАСТИЕ В ЗАПРОСЕ ЦЕН (Форма 1)» изложить следующим образом:

expresses its consent to meet the terms and conditions indicated in the request for quotation notice and request for quotation documentation, terms and conditions of the contract provided for by PART 2 “Main Requirements for the Contract” of Volume 1 of the request for quotation documentation and offers the goods (work, services) according to the Appendix to the Application “Technical Proposal”.

Hereby we give our consent and confirm that we have obtained consents of all affected and concerned persons indicated in Table 2 “Information of the Chain of Owners Including Beneficiaries (Including Final Ones)” of Form 2 “Questionnaire of the Request for Quotation Participant” which are required in accordance with the applicable law of the Russian Federation to process the provided information by the customer (request for quotation organizer) and disclosure by the customer of the information in full or in part to competent state authorities (including Federal Tax Service of the Russian Federation, Minenergo of Russia, Rosfinmonitoring and Government of the Russian Federation) and for subsequent processing of such information by such authorities.
If we are acknowledged as the request for quotation winner, we shall assume the obligations to conclude the contract in accordance with the request for quotation documentation and terms and conditions of our request for quotation application and to provide the updated information on the chain of owners including beneficiaries (including final ones) in case if such information was changed after we had filed the request for quotation application but before the contract conclusion. If no changes were made, we assume the obligations to provide the certificate of no change. The certificate evidencing that the information is up-to-date will be signed and provided by us not earlier than 5 (five) days before the contract conclusion (by the both parties).
We have been notified and agree with the condition that if we provide unreliable information, we could be debarred from participation in the request for quotation and in case if the unreliability of the information provided by us is revealed after conclusion of the contract with us, such contract may be terminated.
If our request for quotation application takes the second place and the request for quotation winner is recognized as having avoided conclusion of the contract with the customer, we shall sign this contract in accordance with the request for quotation documentation and terms and conditions of our request for quotation applications.

that the information on us will be included in the respective public register of unfair suppliers for two years in the following cases:
if we:
are acknowledged as the request for quotation winner and avoid the contract conclusion;
are the participant taking the second place provided that the request for quotation winner has avoided the contract conclusion and so on if we take the third and subsequent places (if the respective ranking is carried out) and avoid the contract conclusion;
are the only request for quotation participant who has filed a request for quotation application or the request for quotation participant who has been acknowledged as the only request for quotation participant and avoid the contract conclusion;
are the participant with whom the contract shall be concluded and refuse from provision of the security for the contract performance, if such requirement is established in the request for quotation documentation;
if the contract concluded with us based on the results of this public request for quotation is terminated by the court decision or by agreement of the parties due to a material breach of the contract committed by us.
In accordance with the instructions obtained from you in the request for quotation documentation the information on the essence of our proposals in this request for quotation is given in the following documents being an integral part of this request for quotation application:

INSTRUCTIONS ON FILLING IN
These instructions shall not be reproduced in the documents prepared by the request for quotation participant.
The request for quotation application shall be on the letterhead of the request for quotation participant.
The request for quotation participant shall assign the date and number to the request for quotation application in accordance with the document circulation rules accepted by it.
The request for quotation participant shall indicate its full name (and its legal form of organization) and legal address.
The request for quotation participant shall list and indicate the number of pages of each of the documents attached to the request for quotation application and describing the essence of the quotation of the request for quotation participant according to Clause 14 of Section 4 “Request for Quotation Information Card”.
The request for quotation participant shall be recommended to sign and seal the request for quotation application following the procedure as stated in Clauses 3.5, 3.6, 3.9
